Select1 Volleyball Club Release Form

The undersigned being the parent and/or legal guardian of the player named above, hereby waives, releases and agrees to hold
harmless Select1 Volleyball Club, coaches, and staff as well as any league sponsored activity or practice connected with such
program or event from any rights and claims or damages to property and injuries to persons whether based upon negligence or
other theory of law which may arise out of or in connection with this guardian assures the league / practice that they have
adequate medical insurance available and if necessary,  and will be responsible for any such medical expenses.

June  7 & 8      ____ Ages 10-14  3:00-5:00pm. Cost is $85.

June 21 & 22   ____ Ages 15-18  3:00-5:00pm. Cost is $85.

July 12 & 13    ____ Ages 15-18  6:00-8:00pm. Cost is $85.

July 19 & 20    ____ Ages 10-14  3:00-5:00pm. Cost is $85.
Aug 16, 17, 19 & 20 ____ Ages 11-14 3:00-5:00pm.  **Special camp for players wanting to try-out for their school team. Cost is $165.

StoneBridge Church is located at the end of Research Forest Dr just before Branch Crossing.  Church can also be accessed thru
a gate on FM 1488.  Press the red call button on the access pad post next to the gate.  Player must bring knee pads, water bottle,
and wear sport clothing.  Parents must sign player in on the first day of each camp.
